Informed Consent & Client Agreement
Please read before commencing therapy. This agreement explains the nature and limits of the services offered by Adelaide Strategic Therapy, client choices and responsibilities, privacy and confidentiality boundaries, fees and cancellations, clinical hypnosis, Hypnoflow, and urgent-support limits. Rebecca Jane (Beck) Elliott provides Strategic Psychotherapy and Clinical Hypnotherapy for adults aged 18+ within her training, professional scope and insurance arrangements. She is not a registered psychologist, psychiatrist, medical practitioner or medical doctor. Therapy and hypnosis are collaborative; participation is voluntary, client autonomy remains, and no particular outcome, cure or timeframe is guaranteed. Hypnoflow may be used for clinical notes and, where discussed and consented to, therapeutic hypnosis recordings. Clinical records are retained for at least seven years. This is not a crisis or emergency service. A minimum of 48 hours’ notice is required to cancel or reschedule; late cancellations and no-shows are charged at 100% of the applicable session fee, subject to applicable law and discretion in exceptional circumstances. For anxiety-focused work, the Six-Session Strategic Therapy Program includes one 90-minute Initial Strategic Therapy Session and five 60-minute Ongoing Individual Therapy Sessions. The total program fee is $1,290, an $80 saving compared with booking the six sessions individually. Three instalments of $430 are available. If the initial session has already been completed and paid at $245, that payment forms part of the first instalment: a $185 top-up is followed by two instalments of $430. You may pause or end therapy at any time, subject to fees already incurred and applicable program terms. Electronic acceptance through Cliniko may be retained as evidence of consent and agreement.
1. Practice, scope and eligibility
Adelaide Strategic Therapy is provided by Rebecca Jane (Beck) Elliott, Strategic Psychotherapist & Clinical Hypnotherapist, ISPA Member #646. Services are strictly for adults aged 18 years and over. Beck provides services within her training, professional scope and insurance arrangements. She is not a registered psychologist, psychiatrist, medical practitioner or medical doctor. Adelaide Strategic Therapy does not provide medical diagnosis, prescribe or change medication, provide psychiatric treatment, or replace medical, psychological or other regulated healthcare where that care is indicated. You should continue appropriate care with your GP, psychologist, psychiatrist or other health professional where relevant.
2. Clinical hypnosis and informed consent
Clinical hypnosis may be offered where it is suitable and consistent with your goals. Hypnosis is a collaborative process involving focused attention, absorption and responsiveness to therapeutic suggestion. It is not sleep, mind control or a loss of personal agency. You remain able to hear, think, choose, communicate and end the exercise. Participation is voluntary: you may ask questions, pause or stop at any time. You will not be required to disclose information you do not wish to disclose. Experiences vary and no particular depth of hypnosis or therapeutic outcome can be guaranteed. Therapy and hypnosis can be helpful, but individual responses vary; Adelaide Strategic Therapy does not guarantee a cure, a specific result, or that progress will occur within a particular timeframe.
3. Six-Session Strategic Therapy Program
For anxiety-focused work, Adelaide Strategic Therapy may recommend the Six-Session Strategic Therapy Program. It includes one 90-minute Initial Strategic Therapy Session and five 60-minute Ongoing Individual Therapy Sessions. Recommendations remain individual, and the program is not a promise of a particular outcome. The total fee is $1,290, which is an $80 saving compared with the standard individual-session total of $1,370. Three instalments of $430 are available. If you have already completed and paid $245 for the initial session before joining the program, that payment is credited as part of the first $430 instalment. You then pay a $185 top-up, followed by two further instalments of $430. The total amount payable remains $1,290. All six sessions should ordinarily be used within 24 weeks of the Initial Strategic Therapy Session. An appointment cancelled or rescheduled within 48 hours of its scheduled start time, or not attended, is charged at 100% of the applicable fee. A late-cancelled or missed program session counts as used and is not replaced. You may pause or end therapy at any time, subject to fees already incurred and the cancellation policy. If you end the program early, completed sessions and late-cancelled or missed sessions are charged at the standard individual rates of $245 for the Initial Strategic Therapy Session and $225 for each Ongoing Individual Therapy Session. Any remaining prepaid balance is then refunded. If the standard fees incurred equal or exceed the program payments received, no refund is due and any incurred but unpaid amount remains payable. Nothing in these terms limits rights available under the Australian Consumer Law.
4. Privacy, communication and urgent support
Clinical records are maintained using systems including Cliniko and Hypnoflow and retained for at least seven years from the last consultation. Hypnoflow may support clinical note-taking and, where discussed with you, consented hypnosis audio recordings for therapeutic homework or tasking. Email, SMS and other electronic communication may be used for appointments, forms, invoices, reminders and agreed resources, but are not monitored continuously and should not be used for urgent or crisis support. Adelaide Strategic Therapy is not a crisis or emergency service and does not provide 24-hour crisis response, emergency assessment or emergency psychiatric care. If you are in immediate danger or require urgent mental-health or medical support, contact 000, Lifeline on 13 11 14, SA Mental Health Triage on 13 14 65, or the appropriate local crisis service.
5. Client responsibilities, pausing and ending care
As a client, you agree to provide accurate and relevant information for safe and appropriate care; attend free from intoxication or impairment that would make participation unsafe or ineffective; raise concerns about therapy, hypnosis, recordings or tasking; use between-session exercises and recordings only as directed; and not listen to hypnosis audio while driving, operating machinery or doing any activity requiring full attention. You remain responsible for your decisions about medical, financial, legal, relationship or other significant life matters and should seek appropriately qualified advice where required. You may pause or end therapy at any time, subject to fees already incurred and applicable program terms. Beck may recommend pausing, ending or referring care where the service is outside scope, no longer clinically appropriate, there are safety concerns, the therapeutic relationship is no longer workable, or another service is more appropriate.
6. Cliniko consent declarations
By signing or electronically accepting this agreement, you confirm that you are 18 years of age or older; understand Beck Elliott is a Strategic Psychotherapist and Clinical Hypnotherapist and not a registered psychologist, psychiatrist or medical doctor; consent to clinical hypnotherapy; consent to Hypnoflow for clinical note-taking and, where discussed, hypnosis audio recordings for therapeutic homework or tasking; understand Adelaide Strategic Therapy is not an emergency or crisis service; accept the 48-hour cancellation policy; acknowledge access to the Privacy Policy; and confirm you have read, understood and had the opportunity to ask questions about this Informed Consent & Client Agreement. Electronic acceptance through Cliniko, including required declarations and submission of your name and date where configured, may be retained as evidence of consent and agreement.
7. Adult eligibility and telehealth location requirements
Adelaide Strategic Therapy provides services strictly to adults aged 18 years and over. Services are not available to minors. For telehealth appointments, you must be physically located in a jurisdiction in which the practice is permitted and insured to provide services. Adelaide Strategic Therapy does not provide services to a person who is located in the United States of America or Canada at the time of the appointment. You may be asked to confirm your location at the beginning of a telehealth session.
8. Fees, payment and cancellation
Current service fees are set out in the Adelaide Strategic Therapy Client Fees & Program Terms and/or booking confirmation. Payment is due in accordance with the booking or program terms provided to you. A minimum of 48 hours’ notice is required to cancel or reschedule an appointment. Cancellations made with less than 48 hours’ notice and non-attendance/no-shows are charged at 100% of the booked appointment fee. This reflects clinical time reserved exclusively for you and the limited opportunity to offer that time to another client. The policy is subject to applicable law, including the Australian Consumer Law, and any discretion exercised by the practice in exceptional circumstances.
